The practice has a contract with North Yorkshire & York Primary Care Trust to provide:-

Essential Services

These include

 

i)       The treatment of patients who are ill or believe themselves to be ill, with conditions from which recovery is generally expected, for the duration of that condition, including relevant health promotion advice and referral as appropriate reflecting the patients Choice wherever practicable.

ii)       The general management of patients who are terminally ill.

iii)       Management of chronic disease in the manner determined by the practice, in discussion with the patient.

 

Additional Services

These cover

 

·          Cervical screening

These are done every 3 years between the ages of 25 and 49, and then every 5 years until the age of 65.  Our nurses are fully trained to perform this test.

 

·          Contraceptive services

These are available from both the nurses and the doctors. Emergency contraception is effective if taken within 72 hours, so if you require this, it is essential to make this clear when booking an appointment.

 

·          Vaccinations and immunisations including the primary courses of immunisations for children and the annual provision of influenza vaccination for those in the relevant at risk groups

 

·          Child health surveillance

We are fortunate to have our Health Visitors working alongside us in the Clifton Health Centre, and we have a weekly Baby & Child clinic.

 

·          Maternity services

We have an attached midwife, who runs an Antenatal Clinic every week at the Clifton Health Centre, and we offer full care during the pregnancy.

 

·          Minor surgery for minor conditions such as cysts, warts and verrucae, and various skin lesions, and a range of joint and tendon injections.

 

Enhanced Services

·          Warfarin Monitoring - In conjunction with York District Hospital we provide a monitoring service for those patients who are on anti-coagulation medication such as Warfarin.

 

·          Family Planning coil fitting - We provide a service for fitting, monitoring, checking and removing intra-uterine contraceptive devices (coils)

 

 

·          Minor Injuries - We provide a minor injury service for patients with simple injuries requiring immediate treatment or injuries or wounds over 48 hours old.

 

·          Multiple Sclerosis - Additional help for those patients who have multiple sclerosis.

 

 

·          Drug Dependency - Care for patients who misuse drugs

 

·          Chronic Arthritis drug monitoring - We also provide a shared care monitoring service for some patients who are under consultant care and on specialised medication for chronic arthritis problems.

  

·          Specialised Care for Mental Health Problems – We have a specially attached Mental Health Worker and a Counsellor.

 

All our care is designed to be delivered within the national targets of patients being able to see a primary care professional, usually a nurse, by the end of the following normal working day or a GP within two normal working days. This will not necessarily be a named GP.
