HOW TO REGISTER AS A PATIENT

If you wish to register as a patient you should bring in your medical card which you will be required to sign indicating that you wish to register. As medical cards are being phased out you may no longer have one, if so, or you are unable to find yours, an alternative registration form will be supplied.

We will also ask you to complete a ‘New Patient Questionnaire’ to provide us with some basic health information about you before we receive your full medical records from your previous doctor.

All patients are now registered with the practice rather than an individual doctor although you have the right to express a preference for a particular doctor which will be noted in your records.
